The applications below can only be completed once your production company has bound insurance coverage with Sutton.

Please complete and submit one of the two Cast Insurance applications below as advised by either the producer or the production company’s insurance broker/agent. Once submitted, the information cannot be changed, and a Sutton Entertainment underwriter will review and determine your eligibility for Cast Coverage. You can save and exit the application at any time and finish it at a later date. Please note that the Cast Insurance Medical Certificate requires an examination conducted by either a Sutton-approved physician (please inquire with the production company’s insurance broker/agent) or a physician who is not your personal physician.
